Introduction: In Iran, for people who have health insurance, an insurance package for basic dental services is also available. Nevertheless, unmet dental needs have been reported in the country. The aim of this study was to determine the extent of use of insurance package for basic dental services among attenders of health centers of Mashhad in 2011.
Methods: In this cross-sectional study with cluster sampling, a total of 1200 attenders to health centers of Mashhad were interviewed and variables such as age, level of education, the name of health insurance, and insured dental services that was received in the year prior to study were evaluated using SPSS software with chi-square and Mann-Whitney statistical tests.
Results: Of 1200 people studied, 1094 subjects (91%) had health insurance coverage. 258 subjects (23.5%) had used basic dental insurance services in the past year. Women received significantly more services than men (P = 0.03). Type of services received were different depending on level of education (P = 0.001), as in people with less education or illiterate, tooth extraction and dental prostheses were more common. Subjects with 51 years of age and older had less commonly used insurance package for basic dental services compared to other age-groups (P = 0.04). The frequency of dental services received by individuals covered by social security organization insurance, was higher than other insurance agencies (P < 0.001).
Conclusion: Despite the appropriate insurance coverage, the use of insurance package for basic dental services was low. Hence, it is necessary to be examined why usage of insurance package for basic dental services is not welcomed by Iranian people.

Background: For modification of residency programs, improvement of the quality of service to patients and increase in clinical capacity of residents, it is necessary to investigate effective job-related factors. The purpose of this study was to assess job burnout, job satisfaction and fatigue among internal medicine residents of Mashhad University of Medical Sciences. Methods: In this cross-sectional descriptive analytical study in 2014, internal medicine residents were randomly included. For the survey, a questionnaire comprised of four sections including demographic features, job satisfaction, burnout (using the Maslach Burnout Inventory) and fatigue (Epworth sleepiness scale) was used Results: Twenty-one residents (62% men) responded to questions. Mean age of respondents was 33.7 ± 3.4 years. Job satisfaction in most items was below average. In total, 71.4% of respondents suffered severe burnout and 23.8% of respondents had moderate burnout. There were inverse significant relationships between some aspects of burnout and job satisfaction. Mean fatigue score based on Epworth sleepiness scale was 10.8 ± 4.7. Five respondents (23.8%) reported severe fatigue. A significant correlation was found between the number of monthly shifts (workload) and the level of fatigue (P = 0.048, r = 0.430). Respondents who had more severe burnout were those who wroked higher number of monthly shifts, although no significant correlation between these two variables (number of shifts and burnout) could not be found (P = 0.112). Conclusion: It seems necessary that in order to improve the quality of education, increase job satisfaction and reduce burnout among internal medicine residents, revision of curriculum (including balancing of night shifts, enhancement of teamwork among residents of different years, etc.) should be taken into account.

Background: Family medicine is an interdisciplinary medical specialty. In this study, the characteristics of curriculum and training course of this field in different countries is presented and the efforts to establish this specialized field in Iran are described.
Methods: In this descriptive study, the characteristics of the training program, working hours, the amount of income, benefits and duration of program in different countries were evaluated.
Results: This specialty was introduced in 1969 for the first time as the 20th independent specialty in the United States. The purpose of this specialty is training of specialized physicians to provide personal, comprehensive and continuing health care for individual who are covered in the context of family and community. In fact, this field encompasses comprehensive health and medical care delivered by the specialist for all diseases, all body organs and in all ages. This field focuses on health promotion, disease prevention and improvement of lifestyle. Duration of training program of this specialty is between 3 to 5 years in most countries. Canada has the shortest training period which is 2 years.In Iran for the better response to the needs of the public health, the field of family medicine was established by a number of Iranian medicine universities including Mashhad University of Medical Sciences in 2009 for the first time. The training of this specialty in this country is based on a combination of synchronous and virtual learning. Training of this specialty is offered in the form of master of public health (MPH), while a 5-year clinical residency curriculum has been developed though it has not been implemented.
Conclusion: All countries define their required medical expertise with respect to their health policy and strategies as well as needs of healthcare system, economic status and social background. In Iran, according to the reform and evolution of the health system and the necessity of implementation of referral system in healthcare to provide specialized care to different levels of the community, attention to the fully development of the specialty of family medicine is more crucial than ever.

Background: A successful program for patients’ referral from small district medical centers to the public referral hospitals indicates a social development and well-managed healthcare service. This study was done to evaluate the clinical and epidemiological characteristics of pediatric patients referred from district cities to Imam Reza hospital, Razavi Khorasan province as well as assessing the status of the patients’ referral system in 2009-2010.
Methods: This retrospective cross-sectional study was performed in Imam Reza Hospital in Mashhad, Iran. We evaluated 220 patients who had been referred from Razavi Khorasan district cities to our center in 2009-2010. Demographic characteristics, primary and last diagnosis, and vital signs on admission were recorded and analyzed.
Results: A total of 220 patients with mean age of 31± 34 months were recruited in this study. 56% of patients were neonates and infants. There was significant inverse correlation between the length of hospital and the patients’ age as infants aged 0-1 month had the longest duration of hospital stay (6.41± 3.69). The most common primary diagnoses in the origin hospitals included pneumonia (21%), poisoning (15%), and congenital heart disease (11%). These diagnoses were significantly correlated with final diagnoses (P = 0.03, Kappa=0.58). Moreover, a significant correlation between severity of abnormal vital signs and mortality was observed (P = 0.001).
Conclusion: In this study we found that neonates had the longest duration of hospital stay. Referral pattern and the relationship between severity of abnormal vital signs and mortality showed that setting up an appropriate protocol for patients’ referral can reduce unnecessary referrals and consequently decrease healthcare costs.

Risk communication is one of the basic concepts in medicine, particularly in dealing with the community. This concept had entered to medical literature many years ago and focuses on the interactions of medical society with public, from policy makers on the tip of health system pyramid to practitioners with direct contact to patients or lay people. Also some of these items are being routinely used but understanding and scientific application of these principles which are expressed based on this conceptual framework, can deeply influence the recipients of services. Selecting the appropriate person for communicating with public, a proper comprehensive message and using correct channel for transmission are the most important principals which should be considered in risk communication. So, it has been recommended to all specialists of health sector to learn these principals and utilize them as much as possible. This article tries to represent a brief review on risk communication concepts.

As the replacement of missing teeth with implant supported restorations has become more common, the numbers of patients requiring dental implant maintenance as part of their preventive or periodontal maintenance care are also on the rise. Although dental implants are immune to dental caries, peri-implant mucositis and peri-implantitis can occur, just as gingivitis and periodontitis are seen with the natural dentition. Many similarities exist between natural teeth and implant-supported restorations with regard to disease etiology and pathogenesis, diagnosis, maintenance therapy, and the need for surgical intervention; however, some modifications in instrumentation and home care should be considered for patients with implant-supported restorations. When the option of dental implant treatment is first discussed with patients, it is important that they understand that although implants have great advantages, they do not absolve the patient from the responsibility of daily oral hygiene practices or regular recall appointments.
