Background: Improvement of the quality of health services will not be possible without regard to the systematic management of errors and medical malpractice and prevention of their recurrence. The aim of this study was descriptive survey of inserted records in the system of treatment files records proposed in the meetings of the Commission of Article 11 in the sanctions unit of department of Mashhad University of Medical Sciences by the type of offense during the years 2007 to 2015.
Methods: The present study was cross-sectional and retrospective. At first, the paper archive records that available since 2007 in the sanctions unit were examined. Infringement by the Commission of Article 11 includes get money out of tariffs, creation of unauthorized medical institute, use of an ineligible person, offering services without a license, failure to renew the license, interfering with the treatment, product fraud, unauthorized closure and absence of physician. The collected data in Microsoft Excel software 2013 were analyzed.
Results: Investigation of infringements set forth in Article 11 Commission in nine years (2007-2015) had a total of 514 infringements. The highest number of infringements in all these years is related to a payment out of tariffs. The highest number of infringements was in 2010 (88 cases) and the lowest was in 2013 (25 cases). 9-year statistics of the Commission Article 11 show that most of the complaints belonged to receiving money out of tariffs and providing services without a license (274 and 70, respectively). Conclusion: Amending regulations and determination of allowed limits of activity of practitioners in each field after health development plan have an important role in reducing crimes such as providing services without a license and getting money out of tariffs.

The Impact of Knowledge Creation Process on Organizational Innovation in Mashhad Hospitals: The Mediating Role of Organizational Learning

Background: Delivering healthcare to patients is a very complex endeavor that is highly dependent on information. Healthcare organizations need to gather and analyze data and transform it into information and knowledge to make useful decisions. Furthermore, the hospital is one of the most interactive working environments and includes interaction between nurses and patients, nurses and doctors, and doctors and patients. All these interactions can lead to human learning and experimentation. Knowledge management and organizational learning play an important role in organizational innovation, so that innovation based on knowledge and organizational learning is evident. This study was performed to investigate the effect of knowledge creation process on organizational innovation in Mashhad hospitals by the mediating role of organizational learning.
Methods: In this cross-sectional descriptive analytical study in 2016, the data gathering method was library-fieldwork. Statistical population of this study was the staff of five Mashhad hospitals (two public hospitals and three private hospitals) which include practitioners and nurses in various sections of hospitals. To determine the sample in this study, the stratified random sampling was used and the sample size based on Cochran's formula was 113. Data analysis was performed with Smart PLS and for statistical analysis, structural equation modeling and T-value test were used.
Results: 50.4% of respondents were men. In terms of education, most participants (57 people) had graduate level. Most people (42.5%) had more than 15 year work experience. The results indicated that knowledge creation process had a structural and significant positive impact on organizational innovation at 95% confidence level (0.361) and an indirect impact on organizational innovation through organizational learning capability at 99% confidence level (0.399). Furthermore, that knowledge creation process had a structural and significant positive effect on organizational learning at 99% confidence level (0.732) and organizational learning had a structural and significant positive impact on organizational innovation at 99% confidence level (0.546).
Conclusion: Regarding to the research results, it is expected that organizations can develop the innovation and knowledge creation process by the help of the development of social networks, work teams and knowledge creation process.

Background: One of the most important professional problems for doctors is patient grievances and complaints to the judiciary and its consequences in their personal and professional lives. Despite the efforts of doctors and medical staff in service delivery to patients and specific sanctity of the field, complaint rate is growingly increasing. Advances in obstetrics and gynecology have increased expectations of human society. Therefore, the present study aimed to investigate the incidence of medical errors in the field of obstetrics and gynecology in accordance with disciplinary court records in Mashhad Medical Council in the years 2013-2014.
Methods: This is a retrospective and cross-sectional study conducted based on census. In this study, all records of complaints in the field of obstetrics and gynecology registered in Disciplinary Court of Mashhad Medical Council was investigated from the beginning of 2013 until the end of 2014. Data included plaintiff’s gender, education, complaint topic, the organization referring the dossier and the type of decision.
Results: Between 2013 and 2014, 701 complaints were registered in Disciplinary Court of Mashhad Medical Council among which 135 complaints were related to the field of obstetrics and gynecology. 17.8% of plaintiffs were men and 82.2% of them were women. Of the 135 complaints of obstetrics and gynecology, most of them related to the death of children (25%), natural childbirth complications in infants (15%) and maternal mortality (10%), respectively. Most referral litigation cases of obstetrics and gynecology in the years 2013-2014 related to private plaintiffs (47.4%). According to expert bodies, 69% of plaintiffs received a sentence of acquittal and 11% were found guilty. In addition, consent was obtained from 2.2% of plaintiffs and 17.8% of files were archived according to experts.
Conclusion: According to the complaints of obstetrics and gynecology and determination of their causes, following practices are essential in order to reduce complaints and increase patient satisfaction: improved doctor-patient relationships, doctors paying more attention to symptoms, tests, medical history and condition and pregnant mothers’ statements, detailed examination of the pregnant mother in late pregnancy (the individual being under the supervision, monitoring, NST test and checking the fetal heart rate) and the presence of a midwife bedside pregnant women in late pregnancy.

One of the issues seriously considered in hospital management is filing medical images. With today's achievement in medical sciences and computer networks, old stereotypes used to take pictures of radiology and the cost of buying and maintaining these films are no longer essential. Picture archiving and communication system (PACS) transmits, stores, retrieves and displays medical images at different points. PACS in medical systems includes necessary software and hardware used to control and manage medical images.
Operation of this system results in reduced storage costs associated with archiving, imaging films storage and communication with hospital information system (HIS) and also provides ability for remote training and diagnostics. Also, by using the teleradiology, it helps physicians around the world to achieve the same and updated information at the same time. As a result, the expansion of its application will be vital in the promotion of public health.
