Background: Publication of articles about problems and setbacks against medical careers has been of less interest in Iran. The aim of this study was to present the viewpoints of medical experts about the journal format, contents and quality of the articles that will be published in the Journal of Mashhad Medical Council. Methods: This descriptive qualitative study was conducted during a meeting in May 2013. A focus group comprised of academic staff of Mashhad University of Medical Sciences (MUMS) and members of the Medical Council of Mashhad (MMC) participated in this meeting. The participants presented their views with lectures and also wrote their views by answering to a pre-designed questionnaire. Results: In total, opinions of 45 medical experts were collected. 85% of participants were male with a mean (SD) age of 49 (7) years old. 76% of the participants were academic staff of MUMS. Participants' opinions were categorized in two areas: (a) the structure of the journal, (b) the scope of the journal. For improvement of the journal structure, using professional editorial team, reducing printing costs, establishment of the journal website (22%), obtaining feedback of the MMC members (22%), and publication of scientific articles (22%) were the most frequent recommendations. Most recommended topics for the articles were: teaching medico-legal laws (60%), providing practical and educational articles about medical professionalism (56%), and educating medical ethics (31%). Conclusion: Medical careers are involved with high loads of work. Hence, they have less time to read and update themselves about their job related issues including medico-legal issues. They may also be less updated about new findings in medical science. Journal of Mashhad Medical Council should work as a scientific platform with collaboration of medical staffs and MMC members to disseminate necessary knowledge for medical community. Furthermore, it should address occupational problems of medical community with research-based perspective.

Background: Human resources are exposed to the scourge of mental pressure caused by organizational factors which can affect job satisfaction. This study was designed to determine “the relationship between job satisfaction and levels of Burnout in clinical staff working at hospitals in the city of Mashhad”. Methods: In this descriptive-analytical study, the sample size was 660 medical personnel in hospitals of Mashhad University of Medical Sciences which were selected with stratified, cluster and in the final phase with simple randomized sampling methods. They completed the research questionnaire which included demographic information form, the questionnaire of job satisfaction and the Maslach Burnout Inventory (MBI). Data were analyzed using SPSS version11.5 with the appropriate statistical tests. Results: According to 3 factors of MBI, 38.6% of subjects reported low levels of emotional exhaustion, 58% low levels of depersonalization and 62.9% low levels of personal accomplishment. 42.1% of the medical staff had low, 54.8% had moderate and 3% had high levels of job satisfaction. There was a significant inverse correlation of emotional exhaustion and depersonalization with job satisfaction as medical staff that had higher job satisfaction reported less emotional exhaustion and depersonalization (P<0.05). High personal accomplishment was more reported form employees who were relatively satisfied with their job (P<0.05). Conclusion: Job satisfaction in staff of hospitals was found to be moderate. The burnout based on emotional exhaustion and depersonalization was low but personal accomplishment was also low (risk factor). Improvement of job satisfaction may reduce emotional exhaustion and depersonalization in medical staff.

Background: Physicians play a key role in the effectiveness of the health system in each country. Providing favorable job conditions for physicians, including appropriate payment system increase the quality of medical services. The aim of this study was to compare general physicians’ (GP) income and taxation system in Iran and other countries. Methods: In this study, GP's income in Iran and 16 other countries was compared with per capita income in these countries. Data was extracted from peer reviewed literatures and reliable websites. Sources for GP's income in Iran were Guideline of Family Physician Program and Referral System in Urban Areas, Version 02 and Guideline of Implementation of Family Physician Program and Health Insurance in Rural Areas, Version 13. The per capita income considered for each country was gross domestic product based on purchasing-power-parity (PPP) according to International Monetary Fund statistics. Results: Annual income of GPs in the United States ($98,000), Canada ($92,642), Australia ($80,532) and Japan ($71,688) were the highest and in Romania ($4,444), China ($5,675); India ($8,180) and Iran ($12,160) were the lowest. Considering the ratio of annual income of GPs to PPP, physicians in Japan (236%), Canada (224%), India (211%) and the United States (195%) had the highest and physicians in Romania (47%), Czech Republic (69% ), China (75%) and Kuwait (96%) had the lowest ratio of income. In this respect, although GPs in India earned one of the lowest annual incomes, they were in one of the top countries according to the ratio of income to PPP. Conclusion: Irrational low incomes for physicians may result in decreased job satisfaction, long working hours, increased risk of medical errors or illegal earnings and consequently decrease in quality of health-care services provided to patients. GPs additional and illegal earnings in the countries with higher income are low and quality of services provided to patients is high. A rational and flexible payment system according to existing conditions can help physicians to achieve their rights in Iran and therefore increase quality of health-care services.

Background: Along with the advancement of medical sciences and improvement of medical services, public awareness about their rights against medical system has been increased. Annually, a significant number of claims are referred to the disciplinary court of Mashhad medical council (MMC). The aim of this study was to investigate claims submitted to this office. Methods: This study was done on 334 claim files submitted to the disciplinary court of MMC in 2011. Data were recorded on a pre-designed checklist. Data included gender of plaintiff, subject of claim, relationship of plaintiff with patient, gender of defendant(s), defendants’ field of expertise, the judgment of board of experts, board of magistrates and board of appeals. Results: In total, 285 (85%) files which were finalized through judgment process were enrolled. Among the plaintiffs, 69% were male, 25% females, 4% MCC and 2% Mashhad University of Medical Sciences (MUMS). Regarding field of expertise, most claims were related to gynecologists (16%), dentists (15%), general practitioners (13%) and orthopedics (12%), respectively. According to the judgment of board of experts, 65% of defendants acquitted, 30% were sentenced of malpractice and 5% of cases were closed or plaintiff acquiesced. Most of the proven malpractices were related to general practitioners (21%), dentists (21%), gynecologists (9%), orthopedics (8%) and ear, nose and throat specialists (6%), respectively. Conclusion: Claims about medical community is an important issue requiring special attention. Physicians should pay enough attention to their patients’ problems and should consider professional commitments. In addition they should study and update themselves about medico-legal laws to reduce number of claims and possibility of being sentenced.

Background: Medical Council as well as other organizations provides welfare services to its members. The aim of this study was to investigate the pattern of members’ interest and their use of services and amenities offered by the Mashhad Medical Council (MMC). Methods: In this descriptive cross sectional study, data from the MMC Welfare Club in 2012 were collected and assessed. Data collected included the gender of members and type of services offered. Data were analyzed using Excel software. Results: Overall, 38,194 person-services were used from the MMC welfare club in 2012. Among the services offered, fast food and coffee shop (28%), catering and restaurants (20%), hypermarket (14%), clothing store (13%), swimming pool and sports club (3%) were the most common amenities used by the medical community. Moreover, members had less interest in the use of Internet services (0.3%), computer and audiovisual equipment stores (0.3%), Interior Decoration (0.3%), sports car accessories (0.04%), musical instrument store (0.02%). Pattern of use of services was different among male and females. In this regard, male members used sports car accessories (male to female ratio: 446%), Hypermarket (251%) and Internet services (207%) in higher ratios while females used wedding supplies store (0%), Interior Decoration (21%) and cosmetics shop (21%) in higher ratios. Conclusion: In this study, it was found that among welfare services offered by the MMC, members were mostly interested in coffee shops, restaurants, catering and hypermarkets. Medical Council, in addition to making deal with high quality amenities and welfare centers, should widely inform the members and identify their interests

Background: Continuing education education (CME) includes all educational programs after graduation in order to maintain and improve the skills and update medical knowledge. The purpose of this study was to evaluate the educational programs held by the CME department of Mashhad University of Medical Sciences. Methods: In this descriptive cross-sectional study, CME programs held in 2011 and 2012 were evaluated. Data including the type of CME program, target group and number of participants were collected. In this study only the data of groups which were targeted for most programs were analyzed. Results: A total of 597 CME programs including 87 compiled programs, 36 seminars, 107 workshops, 105 conferences, 243 periodic conferences, 9 professional short courses and 10 congresses were held in 2011 and 671 CME programs including 100 compiled programs, 28 seminars, 90 workshops, 151 conferences, 282 periodic conferences, 10 professional short courses and 10 congresses were held in 2012. In the majority of programs, general physicians (GPs), internal medicine specialists, gynecologists, oncologists, general surgeons and nurses were the target group. Considering the ratio of participants to the programs for each target group, nurses, GPs, pharmacists, dentists and midwives participated in the highest rates. On the other hand, thoracic surgeons, oncologists and radiotherapists had the lowest rate of participation in the programs. Conclusion: Conducting CME programs, in addition to detailed planning, scheduled implementation and precise scoring for each group; requires accurate determination of target groups and timely informing to them. Revising the definition of target groups, rapid informing of each program to target groups and moving towards e-learning should be the goals of the department of CME.

Background: During the recent years, Iran’s Ministry of health and medical education has considered the strategy of outsourcing of medical services to make rational government size, strengthening accountability of the system and improving efficiency of the health services. The objective of this study was to evaluate effectiveness and efficacy of outsourcing of pharmacy services of health care centers to the private sector with the approach of evidence-based policy making. Methods: In this descriptive cross-sectional study, 7 different cities were randomly selected. Data were collected through semi-structured interviews, reviewing document of Financial and Medicinal Affairs Unit. Formal and content validity of the instruments were confirmed by expert panel and reliability confirmed with Test-retest and Cronbach's alpha. Data were analyzed using T-test and Wilcoxon rank test. Results: By outsourcing of drug services to private sector, it was found that large amount of expenditures in personnel costs was saved. Income after the assignment was shown to increase about 2 times (P<0.001). The mean number of prescriptions, showed 48% increase (P<0.01). Authorities were more satisfied with the results of this outsourcing. Conclusion: This study showed that there was an improvement in effectiveness and efficacy of pharmacy services. Hence, it was found that outsourcing of medical services to private sector was successful.

Analysis of the Votes in the 6th Election of Board of Directors of Mashhad Medical Council in 2013

Background: Medical Council is the largest and oldest non-governmental organization in Iran. This study was aimed to analyze the votes in the election of board of directors of Mashhad Medical Council (MMC) in 2013. Methods: This descriptive-analytic study was conducted on the votes of the voters in the election of MMC. Analysis was exclusively performed on the votes of physicians. Collected data included the number of votes, gender, age, having general doctorate or specialty certificate, being member of the academic staff and governmental employment. Statistical analysis was done using SPSS software. Results: A total of 2706 members including 1603 physicians, 450 dentists, 224 pharmacists, 98 doctorates of laboratory sciences, 301 licensed experts and 90 midwives participated in the election. Proportion of the number of voters to the total members of the MMC was 25% and in different groups was 25% in physicians, 33% in dentists, 25% in pharmacists 62% in doctorates of laboratory sciences, 47% in licensed experts and 4% in midwives. Data analysis of the votes of physicians showed that higher age, having a specialty and being member of the academic staff were the effective factors for a person to be elected. However, gender and governmental employment did not show any impact. After considering the interaction of all factors, the only effective factor with achieving higher votes was being member of the academic staff (P=0.001). Conclusion: These results suggest that physicians mostly trust academic members to solve their occupational problems. Participation in the election of MMC was about half of the election of general medical council in England where the voting is performed through posting vote papers (ballots) to members’ address. Since the majority of the healthcare professionals in Iran may be involved in hospital, operating room or being on duty in the day of election, sending ballots to them can lead to increased participation in the election of MMC.
